Job summary

Northern Pines Mental Health Center is seeking a Mental Health Practitioner to join our Children and Families team. This full-time role operates primarily in a school-based setting at Pierz Elementary School, Pierz, MN 56364, with a small in-home caseload.

The aim is to provide mental health services to youth and families referred to the center. The role emphasizes intake, treatment planning, documentation, and group facilitation, working with the clinical supervisor to ensure high-quality care

Qualifications

  • Minimum 2,000 hours supervised work experience in mental health services.
  • Experience and training working with children.
  • Valid driver’s license.

Responsibilities

  • Review referrals and clarify requested services and frequency.
  • Coordinate intake sessions with family and supervisor.
  • Complete and sign treatment plans.
  • Ensure documentation such as diagnostic assessments and progress notes are completed timely.
  • Provide content to clients in individual and group settings.
  • Facilitate and co-facilitate client groups with supervision.
  • Maintain contact with referral sources as agreed.
  • Ensure initial and annual diagnostic assessments are completed.

Job Location: Pierz Elementary School - Pierz, MN 56364. Position Type: Full Time. Salary Range: $24.50 - $24.50 Hourly. Job Shift: Day.

We are seeking a Mental Health Practitioner to join our Children and Families team. This full-time role will primarily operate in a school-based setting, and will also keep a small in-home caseload. The general purpose of this position is to provide mental health services to youth and families referred to Northern Pines Mental Health Center.

Essential Functions
  • Review and promptly respond to referrals. Clarify frequency and type of services referring worker is requesting.
  • Coordinate and conduct intake sessions to include: complete intake paperwork with the family, obtain billing information, authorizations and continued care requests as needed. This will be done in conjunction with the clinical supervisor.
  • Complete a treatment plan consistent with the needs identified in the evaluation, by the family and by the referring worker. Ensure the clinical supervisor signs the treatment plan.
  • Ensure all documentation is completed as mandated (current diagnostic assessments, ROI’s and renewing ROI’s, treatment plans, functional assessments, progress notes within 24 hours of service delivery).
  • Prepare and provide content to clients in an individual and group setting.
  • Facilitate and co-facilitate client groups as agreed upon with the clinical supervisor and colleagues.
  • Maintain contact with the referral source as agreed upon.
  • Ensure a diagnostic assessment has been completed initially and annually thereafter.
  • Conduct transition planning and referrals as needed.
  • Participate in Rule 29 and CTSS Supervision and Consultation at which time progress notes are submitted to the supervisor.
Qualifications
  • Must qualify as a mental health practitioner as defined by the Minnesota Department of Human Services in one of the following ways:
    • Has at least 2,000 hours (approximately two years working full-time) of supervised work experience in the delivery of mental health services.
    • OR holds a master's or other graduate degree in behavioral sciences or related fields.
    • OR holds a bachelor's degree in behavioral sciences or related fields and completes a practicum or internship that requires direct interaction with adults or children served, and is focused on behavioral sciences or related fields.
  • Experience and training working with children.
  • A valid driver’s license.
