Starting Salary Range: $58,911 - $62,079

RADIAS Health has a full-time position available as a Mental Health Practitioner on our Community Treatment Team (CTT). The CTT team provides services to 50 clients in Ramsey County with 5 Team members with 10 clients on each caseload. Mental Health Practitioners will help clients locate, secure, and maintain independent housing in Ramsey County. Mental Health Practitioners will also provide instruction and monitoring of independent living skills to persons served to ensure the person has the supports necessary to manage as independently as possible in an apartment setting.

Hours: Tuesday - Saturday 8 am - 4 pm, 40 hours per week

**This position works onsite at the Rose Center apartments in Saint Paul, Minnesota. **

Duties and Responsibilities:

  • Provide instruction and monitoring on the identified service areas indicated in the Housing Focused Person Centered Plan and/or Professional Statement of Need, or in assessment information available from collateral sources
  • Using person-centered approaches, develop a plan to get and/or keep housing
  • Decide on appropriate interventions or assist the client in obtaining and maintaining the highest level of independence possible
  • Continually monitor client progress and with the clinical team and modifies the support plan as appropriate. The Stabilization Specialist will communicate changes in service needs and recommendations to Program Director, Case Manager, and other people involved with the person's support.
  • Maintain records of progress for each face-to-face meeting and telephone contact with client and other providers involved with housing, such as Economic Assistance and community housing providers.
  • Assist clients to create a plan to work towards service goals in regular meetings with the client.
  • Schedule appointments to see the client to give requested services for the amount of time and frequency agreed upon with the client, and as needed based on the client's immediate needs.
  • Intervene as necessary for emergencies and/or when he/she assesses that the client is unable to manage his/her housing in a reasonable manner. Contact to appropriate emergency services, doctors, staff, and emergency family contacts will be made to alert them of intervention planned or taken.
  • Coordinate and cooperate with other agency and community providers to provide the best possible care for the client, including but not limited to landlords and other community resources. The Stabilization Specialist will complete all necessary Release of Information documents for communication with other providers and keep these current for each client.
  • Assist clients in obtaining and maintaining benefits and necessary assistance in budgeting monthly funds, as well as support needed to retain housing subsidy
  • Participate in support around psychiatric follow up, medication adherence and assistance with other community resources related to the person's mental health needs.
  • Must be able to transport clients in the community.
Requirements
    • Candidate will have a Bachelor's degree in Psychology or Social Work plus 2,000 hours work experience providing services to adults diagnosed with a serious mental illness
  • -OR-
    • No college degree and 4,000 hours supervised work experience in the delivery of services to adults or children with mental illness.
    • Must have car, auto insurance and driver's license.
