Minnesota Mental Health Solutions LLC is seeking a dedicated Practitioner to join our Eden Prairie team in MN. You will provide direct mental health services in a collaborative clinical environment, conducting assessments and developing individualized treatment plans.

The role requires active licensure in Minnesota, a relevant degree, and strong communication and documentation skills. We value trauma-informed, client-centered care and teamwork to improve client outcomes.

Qualifications

• Active licensure or certification as a mental health practitioner in Minnesota.
• Experience providing direct mental health services in a clinical setting preferred.
• Strong knowledge of evidence-based therapeutic modalities and treatment approaches.
• Excellent communication, interpersonal, and documentation skills.

Responsibilities

• Provide direct mental health services and clinical care to individuals, families, or groups.
• Conduct comprehensive assessments, evaluations, and diagnostic interviews.
• Develop and implement individualized treatment plans based on client needs.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
• Collaborate with a multidisciplinary team to coordinate client care and treatment goals.
• Monitor client progress and adjust treatment approaches as clinically appropriate.
• Uphold ethical standards and maintain client confidentiality in accordance with HIPAA guidelines.
