POSITION SUMMARY: The Mental Health Outreach Worker serves as the backbone for the service coordination and will assist clients transitioning from prison or jail back into the community by providing support, guidance, and service coordination. Mental Health Outreach Workers provide the direct outreach to clients ensuring both the overall health of the client and basic needs are met within the program. This includes providing clients with transportation to mandated court cases, speaking on clients’ progress to the court, facilitating group rehabilitation and weekly check-ins, barrier removal, community reintegration via public outings, and discharge planning to ensure successful placement into lower-level care facilities.

DUTIES AND RESPONSIBILITIES

  • Complete initial and updated Needs and Service Plans (NSP’s).
  • Provide a summary of clients for all county meetings.
  • Plan and facilitate weekly group sessions using the provided curriculum.
  • Find and plan fun in-house activities for Clients.
  • Transport to scheduled court appointments (as needed)
  • Meet with each Client 1×1 for 30-60 min to discuss NSP goals, placement, or discharge.
  • Write progress notes based on 1×1 and group interactions.
  • Develop activity calendar monthly.
  • Locate placements for clients.
  • Preplacement interviews as needed, while completing the functional capability form and preplacement appraisals
  • Review client referrals.
  • Meet with County case managers and conservators as needed.
  • Accurately manage any appointments and communicate with the treatment team
  • Report accordingly/crisis response team.
  • Attend weekly team/clinical/1×1 meetings.

QUALIFICATIONS

  • College degree is preferred but not required. Must have a minimum of 45 college units with at least 30 units in psychology, social work, nursing or related social services field of study.
  • Must be at least 21 years of age
  • PPD test for tuberculosis and satisfactory results
  • One (1) year of experience preferred. Understanding of psychiatric illness and its treatment, and knowledge of psychotropic medication.
  • CCL approved FBI and DOJ criminal background clearances.
  • Free from state sanctions
  • CPR/First Aid/BLS certification Previous experience with population to be served and/or work in a similar program setting preferred.
  • Able to read and write clearly and accurately.
  • Act as a team member providing input and reporting observations and concerns regarding client care.
  • Demonstrate the ability to act as a part of a team and in an appropriate manner in a crisis.
  • Ability to work independently and be self-motivated in an often-ambiguous environment.
  • Proven track record with maintaining strict confidentiality and appropriately work with sensitive information/documents.

PHYSICAL REQUIREMENTS

  • Ability to climb stairs multiple times a day and be able to lift and/or carry up to 25 pounds.
  • Ability to sit, rise, kneel, squat, twist, pull bend multiple times a day.
  • Ability to walk with items in hand, grasp and reach as needed.
  • Ability to perform with repetitive maneuvers and manual dexterity.
  • Ability to work in an office environment.
