Job summary

Qualifications

Responsibilities

• Utilizes glucose monitoring to obtain readings and flag deviations
• Provide direct care for veterans with complex needs
• Observe and report changes in patient condition to the nurse
• Document care provided and outcomes in charts
• Perform nursing procedures within scope (enemas, catheterization, wound care)
• Assist with treatment planning meetings and patient self-care promotion
