High Point & Affiliated Organizations is seeking a Medical Nurse Practitioner to provide both outpatient and inpatient care for patients with changing mental status or chronic medical conditions in Massachusetts. You will work with an interdisciplinary medical team and take independent responsibility under the Chief of Medical Services to evaluate, manage, and treat patients.

Qualifications

• Possess and maintain Massachusetts registered Nurse Practitioner credential (unrestricted).
• Certification from American Academy of Nurse Practitioners (AANP).
• Compassionate, sensitive, and possess strong reasoning and thinking skills.
• Strong computer skills, including the electronic health record.

Responsabilités

• Provide outpatient and inpatient care for patients with changes or decline in mental status, chronic or urgent medical conditions.
• Work independently within the medical team with defined responsibilities by the Chief of Medical Services.
• Participate in an interdisciplinary team to deliver care to patients.
• Perform medical evaluations, risk factor assessment, and order tests; attend consultations and appointments.
• Prescribe medications per clinic policy, formulary, and state regulations.
• Consult with physicians on cases beyond the NP scope.
• Document patient data in the electronic medical record and case presentations.
• Ensure smooth communication with other health professionals for effective clinic delivery.
• Provide consultations to program staff to enhance overall quality.
