Job summary

Balanced Wellbeing LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join our Mental Health Professional team. This role focuses on geriatric care, behavioral health integration, and care coordination across skilled nursing, assisted living, and office settings.

The LPN will conduct medication reviews, develop individualized care plans, and communicate with patients, families, and facility staff. Flexible weekday schedule with potential for bonus pay and no weekend work.

Qualifications

• Active LPN certification in state applying for (required).
• Strong knowledge of Chronic Care Management and Behavioral Health Integration.
• Excellent communication and interpersonal skills.
• Ability to prioritize tasks and manage multiple responsibilities effectively.
• Proficiency in electronic medical record (EMR) systems and other relevant software.

Responsibilities

• Provide care in skilled nursing and assisted living facilities daily.

• Conduct medication reviews and assess mental health.
• Utilize psychiatric scales for assessments.
• Schedule visits for additional patient support.
• Monitor adherence to care plans and evaluate outcomes.
• Document patient progress timely.
• Support Behavioral Health Integration (BHI) initiatives.
• Visit patients in nursing homes and complete charting in the office setting.

Position Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team as a Mental Health Professional. This role is ideal for those passionate about working with the geriatric population, supporting behavioral health integration, and providing care coordination for patients in both facility and office settings.

Responsibilities

• Provide care in skilled nursing and assisted living facilities on a daily basis.
• Develop and implement patient care plans using electronic systems, including goal setting and progress monitoring.
• Communicate daily with patients, POAs, facility staff, and families.
• Conduct medication reviews with patients and ask in-depth questions related to their mental health diagnosis.
• Utilize psychiatric scales for assessments.
• Schedule visits for patients requiring additional support.
• Monitor adherence to care plans and evaluate effectiveness.
• Document patient progress in a timely manner.
• Support Behavioral Health Integration (BHI) initiatives.
• Visit patients independently in nursing homes and complete charting in the office setting.

Compensation & Schedule

• Pay: $28–$32per hour.
• Schedule: Monday – Friday, 8:30 AM – 5:00 PM (30-minute lunch).
• No weekends required.
• Additional bonus pay available.
