Job summary

Balanced Wellbeing LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join as a Mental Health Professional, focusing on geriatric care and behavioral health integration. The role involves care coordination in skilled nursing and assisted living settings, with strong emphasis on communication with patients, families, and facility staff.

You will develop and monitor care plans, conduct medication reviews, and utilize EMR systems to track progress while supporting chronic care management

Qualifications

• Active LPN certification in state applying for.
• Strong knowledge of Chronic Care Management and Behavioral Health Integration.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide care in skilled nursing and assisted living facilities on a daily basis.
• Develop and implement patient care plans using electronic systems, including goal setting and progress monitoring.
• Communicate daily with patients, POAs, facility staff, and families.
• Conduct medication reviews with patients and ask in-depth questions related to their mental health diagnosis.
• Utilize psychiatric scales for assessments.
• Schedule visits for patients requiring additional support.
• Monitor adherence to care plans and evaluate effectiveness.
• Document patient progress in a timely manner.
• Support Behavioral Health Integration (BHI) initiatives.
• Visit patients independently in nursing homes and complete charting in the office setting.

Position Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team as a Mental Health Professional. This role is ideal for those passionate about working with the geriatric population, supporting behavioral health integration, and providing care coordination for patients in both facility and office settings.

Responsibilities

• Provide care in skilled nursing and assisted living facilities on a daily basis.

Compensation & Schedule

• Pay: $28–$32per hour.
• Schedule: Monday – Friday, 8:30 AM – 5:00 PM (30-minute lunch).
• No weekends required.
• Additional bonus pay available.

Requirements

• Active LPN certification in state applying for (required).
• Strong knowledge of Chronic Care Management and Behavioral Health Integration.
• Excellent communication and interpersonal skills.
• Ability to prioritize tasks and manage multiple responsibilities effectively.
• Proficiency in electronic medical record (EMR) systems and other relevant software.
• Strong computer skills with attention to detail.
• Compassionate and patient-centered approach.

Compensation details: 28-32 Hourly Wage
