Job summary

Keystone Human Services seeks an Intensive Case Manager to provide individualized, person-directed support in mental health. The role focuses on helping adults access medical, social, educational and other services to foster independent living and community participation.

Qualifications

  • Effective communication and proficient computer skills are required.
  • High school diploma with 12 credits in sociology, social welfare, psychology, gerontology, or other social sciences and 2 years of human services experience with 1 year of direct client contact.
  • Bachelor’s degree in social work, sociology, psychology, gerontology, anthropology, criminal justice, theology, counseling, education, or nursing.
  • Registered Nurse licensure is acceptable.
  • Valid driver’s license with daily access to a privately maintained and insured vehicle.
  • Pre-employment process including clearances and verifications.

Responsibilities

  • Assist adults with mental illness in gaining access to medical, social, educational, and other services.
  • Support outcomes focused on independent living, vocational/educational participation, and adequate social supports.
  • Aim to reduce hospitalization and enhance community participation.
  • Provide individualized, person-directed support in mental health.

Job description

Keystone Human Services is currently seeking Intensive Case Managers to join our team in providing individualized, person-directed support in the area of mental health. Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer.

As an Intensive Case Manager, you will assist adults with mental illness gain access to needed resources such as medical, social, educational, and other services with outcomes focused on independent living, vocational and educational participation, adequate social supports, and reduce hospitalization.

Job Details: Full time, non-exempt position $22.00 per hour.

Minimum Educational Qualifications: (one of the following) High school diploma with twelve (12) credits in sociology, social welfare, psychology, gerontology, or other social sciences and two (2) years of experience in human services with one (1) year of direct client contact. Bachelor’s degree in social work, sociology, psychology, gerontology, anthropology, criminal justice, theology, counseling, education, or nursing. Registered Nurse licensure.

Additional Minimum Requirements: Effective communication and proficient computer skills. Valid driver’s license with daily access to a privately maintained and insured vehicle. Successful completion of the pre-employment process, including clearances and verifications.
