Description

The Intensive Case Manager provides services to outpatient psychiatric clients. They will work with the Community Care Center and home health team to identify and support vulnerable community members. Case management includes providing short term intensive care coordination with partner agencies, housing resources and health care providers. It also includes continued mental health stabilization, assisting clients' success for a given period of time in their community. They will continue to work with some clients for an extended period of time after they are transferred to adult case management services. Group therapist coordinates and conducts group psychotherapy. Elements of this position include clinical assessment of patients, brief psychotherapy, crisis intervention, referral, care coordination, and transition.

Required qualifications:

  • Master's Degree in Mental Health counseling or equivalent.

  • 2 years Acute care or community based mental health work.

Preferred qualifications:

  • Working with and/or a knowledge base of homeless services and/or substance use.
