Job summary

Tufts Medicine is seeking a Mental Health Counselor to join the multidisciplinary patient treatment team. The role supports clinical care under RN supervision and focuses on evaluating and addressing patients’ behavioral health needs.

The position requires a Bachelor's degree in psychology or related field and at least one year of direct patient care experience. It offers a dynamic, patient-centered environment with a rotating schedule across days, evenings, weekends, and some nights.

Qualifications

• Bachelor's degree in psychology, sociology, or related human service field.
• One year of direct patient care experience.

Responsibilities

• Review patient data at shift start to determine status.
• Implement and monitor treatment and behavior plans.
• Read care plan entries to ensure consistent delivery of care.
• Identify scheduled tests or appointments and provide education.
• Meet with patient early to review the day’s schedule.
• Discuss patient mental status and progress with the team.
• Report safety and behavior changes to the RN on shift.
• Provide input to the Nurse Manager/Charge RN when needed.
• Share observations with social workers and family as appropriate.
• Document patient activities and mental status per plan.
• Conduct individual patient contacts to assess needs.
• Identify patient teaching needs and report to RN.
• Identify barriers to discharge and progress toward goals.
• Encourage participation in therapeutic groups and behavior plans.
• Provide patient/family education regarding milieu and goals.
• Participate in crisis intervention per CPI and safety policies.
• Conduct safety checks for patient and unit mood.
