Position Objective

The Licensed Clinician will work in tandem with the primary care team and is responsible for providing brief intervention and consultation to patients in an outpatient setting. Services include crisis intervention, monitoring mental health referrals for patients with higher behavioral health needs, coordinating care for complex patients with mental and physical health conditions, acting as a consultant and resource to the care team on interventions for patients with chronic illnesses and mental health diagnoses, assisting with staff training, functioning as a chronic care self-management support expert, and ensuring timely documentation and data collection.

Patient Care Management/Documentation
  • Collaborate with physicians and mental health providers to deliver comprehensive mental health services across all ages.
  • Develop and implement integrated treatment plans based on clinical experience and knowledge.
  • Maintain a customer-oriented approach, involving family as appropriate, and ensure confidentiality and privacy.
  • Document all care provided according to standards and adhere to practice protocols and regulations.
Consultation and Referral
  • Seek and provide consultations as needed.
  • Perform comprehensive assessments and refer patients to appropriate services within the health system or community.
Patient Education
  • Use teaching strategies to influence patient outcomes and identify educational needs to develop relevant programs.
Clinical Responsibilities
  • Diagnose and treat acute mental and emotional disorders.
  • Provide short-term, comprehensive care, coordinating with other professionals.
  • Refer patients to community resources and develop discharge plans with external supports.
Evaluation
  • Assess risk of suicidal or homicidal thoughts or behaviors.
  • Conduct interviews and coordinate examinations to aid diagnosis and treatment.
  • Evaluate program effectiveness and present case information to the healthcare team.
Educational/Experience Requirements
  • Master's degree in a related field.
  • At least five (5) years of related clinical counseling experience.
  • Experience in art therapy is strongly preferred.
Required Licenses/Certifications
  • Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker Clinical (LCSW-C) from Maryland, or Licensed Clinical Professional Counselor (LCPC) from Maryland.
