Job summary

Hartford HealthCare’s Rushford Center in Glastonbury, CT seeks a licensed social worker to join an interdisciplinary team delivering evidence-based clinical services across the continuum of care.

The role conducts bio-psychosocial assessments, develops care plans, coordinates referrals, educates patients and families, and maintains timely documentation in accordance with licensing requirements.

Qualifications

• Master's Degree in Social Work, Counseling, or related Human Service Field is required.
• Previous experience in a healthcare setting is preferred.
• LCSW, LPC, LMFT, LADC licensure is required.
• Valid Driver's License is required.

Responsibilities

• Conducts comprehensive bio-psychosocial assessments and establishes a care plan.
• Collaborates with interdisciplinary team to assess level of care and coordinates discharge and referrals.
• Utilizes therapeutic methods/interventions per individualized care plan.
• Monitors progress and revises plans to meet patient needs.
• Provides education to patient/family on goals, disease prevention, and treatment adherence.
• Provides direct patient care including case management, care coordination, and crisis intervention.
• Refers patient to appropriate levels of care and resources; maintains timely documentation.
• Independently provide clinical social work interventions to all patient populations.
• Participates in meetings, supervision, and staff education; mentors junior staff.
• Supports hospital/unit programming to meet clinical needs and patient outcomes.
• Works with the team to improve patient experience and cost-effective resource use.
