Job summary

Commonpoint Queens is seeking a Clinical Social Worker to deliver mental health services at a senior center satellite clinic serving adults 50+. You will work closely with a multidisciplinary treatment team including a psychiatrist, NP, social workers, peer advocates, and a nurse.

Supervision and weekly conferences are provided on-site. You will conduct intake assessments, create and monitor treatment plans, and participate in case conferences and ongoing professional development.

Qualifications

• Must hold LMSW or LCSW licensure; experience with older adults preferred.
• Bilingual applicants encouraged.

Responsibilities

• Provide mental health psychotherapeutic services at a senior center satellite clinic, including intake assessments and ongoing individual, family, and group treatment.
• Complete all forms (intake docs, treatment plans, progress notes, and required documentation) within required timelines.
• Participate in case conferences and clinical supervision.
• Provide structured and unstructured engagement sessions; conduct screenings; refer to additional services when indicated.
• Educate and guide senior center staff; assist with crisis management and planning for members and families.
• Collaborate with other agencies addressing senior mental health needs.
• Perform additional duties as needed by supervisor.
• Provide mental health services in patients’ homes when necessary.

JOB DESCRIPTION:

This position would be in our CAPE program, an article 31 outpatient mental health center serving adults 50 years old and older. The clinical social worker will work very closely with a larger treatment team consisting of a Psychiatrist, Psychiatric Nurse Practitioner, Licensed Clinical Social Workers, Peer Advocates and a Registered Nurse. The social worker will receive individual supervision and participate in weekly case conferences involving the entire treatment team. The social worker will be able to attend social work continuing education classes offered on site, free of charge.

RESPONSIBILITIES OF THE JOB

• Provide mental health psychotherapeutic services at a senior center satellite clinic. These services include providing intake assessments, and ongoing individual, family, and group treatment. Engagement activities include informal discussions as well as group engagement.
• Complete all forms, such as intake documentation, treatment plans, progress notes and DFTA required documentation, within the required timeframe.
• Participate in case conference meetings and clinical supervision.
• Provide structured and unstructured engagement sessions; mental health, substance abuse and psycho-social screenings; referrals and linkages to additional services when indicated.
• Provide education and guidance to senior center staff when indicated.
• Assist senior center staff with crisis management and planning for senior center members and their families.
• Collaborate with other agencies and organizations working to address the mental health needs of seniors.
• Additional responsibilities as deemed necessary by your supervisor in response to agency need.
• Provide mental health services in patients’ homes when necessary.

Performance & Organizational Excellence

• Participate in the agency's performance management process by establishing annual goals, completing mid-year and end-of-year evaluations, and engaging in ongoing performance discussions with supervisors.
• Complete documentation, data entry, and required reports accurately and on time in accordance with agency standards, funder requirements, and established deadlines, including updates to agency systems, dashboards, and tracking tools.
• Complete required annual professional development activities to maintain and strengthen clinical knowledge, professional skills, and overall job performance.
• Use program and service data to improve performance by participating in data review and discussion during supervision, department meetings, reporting activities, and other quality improvement efforts, including surveys and feedback activities, as applicable.

EDUCATION, EXPERIENCE, AND REQUIRED

LMSW or LCSW with Interest and competence in working with older adults or LMSW with hours of clinical work and supervision complete. Bilingual therapists are encouraged to apply.

WORKING CONDITIONS:
