Job summary

Lakes-Regional-Community-Center in Sulphur Springs, TX is seeking a counselor to help individuals access medical, social, educational resources and to provide rehabilitative and skills training.

The role includes case management, care coordination, treatment planning, crisis services, and documentation of progress; telemedicine and supported employment are also offered. A Bachelor's degree in a related field and a valid Texas driver's license are required.

Qualifications

• Bachelor's degree in Counseling Psychology, Psychology, Social Work, or Sociology is required.
• Experience or ability to perform case management and care coordination is preferred.
• Proficiency with Microsoft Office and standard computer skills required.

Responsibilities

• Assist individuals in gaining access to medical, social, educational resources.
• Provide rehabilitative and skills training, crisis services, and treatment planning.
• Document client progress and coordinate care across teams.
• Offer telemedicine services and supported employment where applicable.

Job description

Responsibilities includes assisting individuals with gaining access to medical, social, educational, and other needed resources, including provision of rehabilitative and skills training, case management, care coordination, treatment planning, crisis services, as well as documentation of client progress. Also provides supported employment and telemedicine services. Must have a valid Texas driver's license and acceptable driving record, as well as personal liability insurance as required by the state of Texas. Working computer skills including Microsoft Office.

Schedule:

Monday - Friday 8 AM - 5 PM

Minimum Qualifications:

Bachelors or better in Counseling Psychology, Psychology, Social Work, or Sociology.
