Qualifications include a health sciences degree and specialized experience with patient assessments and coordinating care; substitution of education for experience is not allowed.

Qualifications

• Bachelor's or graduate degree in a health-related field from an accredited program.
• One year of specialized experience including conducting patient assessments or monitoring progress; coordinating with medical providers to ensure continuity of care; and participating in a multidisciplinary treatment team.
• Education cannot be substituted for experience for this position.
