Job summary

Evergreen Family Medicine seeks a Qualified Mental Health Associate (QMHA) to join its behavioral health team in Roseburg, Sutherlin, or Myrtle Creek, Oregon. You will work under a licensed professional to support patients, coordinate care, and help implement treatment plans.

The role emphasizes brief interventions, care coordination, and effective documentation within the EMR, while maintaining HIPAA and practice standards in a busy outpatient setting.

Qualifications

• Must hold MHACBO certification or be able to obtain within 30 days.
• Bachelor’s degree in a social science per MHACBO criteria or 3 years of relevant behavioral health experience.

Responsibilities

• Establish trusting relationships with patients and their families while providing support and encouragement.
• Provide brief mental health interventions and warm handoffs.
• Cofacilitate groups with a licensed mental health professional.
• Perform mental health case management as requested by EFM provider.
• Support patients in transition of care from BHU and other settings.
• Maintain updated screenings and research new tools as directed by EFM.
• Record patient encounters in the EMR within 72 hours of contact.
• Maintain HIPAA, QMHA ORS/OARS, MHACBO standards and EFM policies.
