Participates as a member of the mental health team on inpatient and/or outpatient units. Provides direct patient care, including treatment planning, implementation, documentation, milieu safety assessments, and activities such as group activities.

Major Responsibilities:
  1. Serves as a member of the Mental Health Team, supporting care delivery to patients, families, nurses, physicians, and colleagues.
  2. Provides clear, timely communication regarding patient observations and changes to licensed nurses.
  3. Maintains patient safety through safety checks, environmental rounds, and fall prevention strategies.
  4. Uses de-escalation and calming techniques, following restraint policies when caring for restrained patients.
  5. Contributes to a safe and therapeutic environment.
  6. Leads or co-leads group activities, including goal setting and recreational activities.
  7. Assists in medical emergencies as directed.
  8. Records vital signs, height, and weight measurements.
  9. Performs procedures such as EKGs, phlebotomy, and intake/output measurements as assigned.
  10. Documents progress and group notes as directed by licensed nurses.
  11. Supports other medical and psychiatric interventions as needed.
  12. Provides personal care to promote patient independence.
  13. Utilizes interventions to de-escalate patients and participates in restraints when necessary.
  14. Performs safety checks and monitors unit safety, documenting findings.
  15. Reviews and follows treatment plans, providing feedback to the clinical team.
  16. Maintains medical records, organizes charts for admissions, discharges, and transfers.
  17. Attends staff meetings and in-service programs.
  18. Participates in quality improvement initiatives.
  19. Complies with health, safety, and HIPAA requirements.
Position Qualifications:

License/Certification/Education:

  • High school diploma or equivalent.
  • CPR certification required upon hire and every two years.
  • Avade 2 (restraint) training completed within 30 days of hire.

Preferred:

  • Associate's degree.
  • Knowledge of medical terminology and computer skills.
  • Two years of experience in a health-related field preferred.

Equivalent education and experience may be considered.

Physical Demands and Environment:

Work is sedentary, indoors, in a patient care environment.
