Job summary

• One year of experience in behavioral health preferred.

Responsibilities

• Advocate for consumers within the mental health system and community.
• Provide counseling, needs assessment, skills training, advocacy and case management.
• Develop problem-focused, goal-oriented treatment plans addressing consumer needs and anticipated outcomes.
• Provide community support activities to caseload and coordinate with external agencies.
• Maintain consumer records per agency, county and state guidelines; coordinate discharge planning and referrals.
