As a Resident Care Manager (RCM) - RN or LPN with previous leadership experience

We are looking for a Nurse to join our team as our Nurse Case Manager for our small Memory Care unit located within a Skilled Nursing Facility (SNF) setting. Are you passionate about Alzheimer’s, Dementia, and related disorders? Do you want to make a difference in the lives of others? Your nursing skills will be utilized in our Five-Star Resident-Centered quality of care facility, working closely with residents, their families, front-line nursing staff, physicians, healthcare providers, and interdisciplinary teams.

Qualifications

• Must be a licensed RN or LPN for the State of Oregon with strong assessment and care planning skills
• 2–3 years experience working as an RN or LPN
• MDS experience preferred but not required
• RNs with long-term care experience, a passion for working with seniors, and experience in Nurse Supervisor roles are encouraged to apply
• Hands-on training provided to learn Marquis nursing systems and procedures
