Job summary

All American Assisted Living in Raynham, MA seeks an LPN to join our Wellness team for compassionate personal care and daily resident support. This per diem role offers $19.00 per hour and shifts on all shifts, with benefits and opportunities for meaningful work with older adults.

Provide direct care, observe changes, follow care plans, and collaborate with a supportive team to maintain residents' independence and dignity in a warm community setting.

Qualifications

  • Compassionate, patient, and dependable approach to resident care.
  • Ability to follow service plans, safety procedures, and supervision.
  • Clear English communication for care instructions and emergencies.
  • Experience in assisted living or healthcare is preferred but not required.
  • Ability to lift, transfer, or assist residents with mobility as needed with assistance.

Responsibilities

  • Provide respectful and compassionate personal care to residents.
  • Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene.
  • Assist residents with walking, mobility, positioning, and transfers according to their service plans.
  • Provide meal assistance and support residents with dining needs when required.
  • Offer companionship, conversation, reassurance, and emotional support.
  • Encourage residents to participate in community programs, meals, and social activities.
  • Respond promptly to resident requests and emergency-call systems.
  • Observe residents for changes in physical condition, behavior, cognition, mood, appetite, mobility, or daily routine.
  • Promptly report changes or concerns to the appropriate nurse or supervisor.
  • Follow individualized service plans, care instructions, and safety procedures.
  • Provide permitted assistance with self-administered medication management after training.
  • Document care and services accurately and promptly.
  • Maintain clean, safe, and organized resident living areas.
  • Assist with personal laundry and light resident-area housekeeping as assigned.
  • Protect resident privacy, dignity, rights, and confidentiality.
  • Communicate respectfully with residents, families, visitors, and team members.
  • Follow infection-control, resident-safety, emergency, and fall-prevention procedures.
  • Participate in required training, meetings, and in-service education.
  • Complete additional resident-care duties within the employee’s training, competency, and assigned role.

What You'll Do

  • Dependable attendance and punctuality
  • Previous assisted living, memory care, senior care, home care, or healthcare experience is preferred, but not required
  • Previous experience assisting with activities of daily living is preferred
  • Ability to stand, walk, bend, kneel, reach, and remain active throughout the workday
  • Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
  • Ability to successfully complete required background checks, health screenings, and employment requirements
Massachusetts Training Requirements
  • Must complete all Massachusetts-required assisted living personal-care training before independently providing personal-care services
  • Must complete community orientation, competency reviews, and required in-service education
  • Must demonstrate competency in the personal-care tasks assigned
  • Must receive appropriate training before assisting with self-administered medication management
  • May only perform duties permitted by Massachusetts assisted living requirements, community policy, completed training, demonstrated competency, and the resident’s service plan
  • Must promptly report changes in resident condition or circumstances requiring nursing judgment
  • May not independently perform nursing assessments, provide nursing diagnoses, administer skilled nursing treatments, or perform duties reserved for licensed clinical staff
