Job summary

Danbury Senior Living in Broadview Heights, OH, is seeking a Memory Care Coordinator with an LPN license. The role leads the Memory Care team, ensures care is right and residents receive appropriate support, and partners with families for ongoing communication.

Responsibilities include staff supervision, care conferences, dementia training, and collaboration across clinical, life enrichment, culinary, and setting teams to uphold our care standards.

Qualifications

• Must hold an active LPN license in the state.
• Prior supervisory experience preferred.
• Ability to obtain Certified Dementia Practitioner certification on hire.
• Strong reading, writing and communication skills.

Responsibilities

• Lead and oversee Memory Care team and performance.
• Conduct care conferences and ensure appropriate care and staffing.
• Coordinate clinical, life enrichment, culinary and setting functions.
• Provide training on dementia-related topics.
• Host Memory Care support groups and family nights.
• Communicate with residents' families and document care.
• Ensure adherence to 1440 care standards.
