Job Summary

Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person‑centered dementia‑friendly programs that meet the specific needs of the residents within the dementia care community. In alignment with Brookdale’s Resident and Family Engagement philosophy, assists in providing person‑centered programs while engaging residents in meaningful and purposeful dementia care programs within the Brookdale community, enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensuring resident habits, historical routines, interests and preferences are honored in the provision of care and programming.

Essential Functions
  • Supports the implementation of a person‑centered program to meet individual needs and interests of the residents, leading and engaging residents in the 7 domains of well‑being programs – purposeful, intellectual, social, spiritual, emotional and physical, including sensory‑focused small groups for residents who cannot participate in a large group setting.
  • Assists in reviewing individual resident service plans and completing program documentation as required by Brookdale standards or state requirements.
  • Ensures dementia care programs comply with state, federal, and other regulations and meet Brookdale’s mission, policies, procedures and standards.
  • Provides guidance and resources to associates on the importance of programming/engagement and other areas regarding the quality of life for residents.
  • Coordinates and participates in special events in the community including family socials, marketing/sales, and open houses.
  • Communicates positively with residents, families, and community professionals regarding the Brookdale dementia care program and its benefits; communicates with families about residents’ personal needs and successes.
  • Assists the manager or coordinator with departmental quality improvement initiatives including the Brookdale Excellence Standards Tool (BEST).
  • Supports planned programming events outside the community, may include driving a community motor vehicle.
  • Assists with the volunteer program, including training and supervising volunteers.
  • Performs personal care, housekeeping and dietary responsibilities as needed to ensure service quality is maintained according to Clare Bridge Program standards.

Compensation and Program Details

Base pay in range will be determined by applicant’s skills and experience. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

Brookdale offers to advance fees for naturalization (Form N‑400) application costs, up to $725, less applicable taxes and withholding, for qualified associates who have been with us for at least a year.

Qualifications
Education and Experience

High School Diploma or equivalent required. Associate’s Degree preferred. Minimum one year of previous experience with older adults and persons with dementia preferred; or equivalent combination of education and experience.

Certifications, Licenses, and Other Special Requirements

Must have a valid driver’s license and a current 3‑year driving abstract with no more than one moving violation as a condition of employment.

  • Use hands and fingers to handle or feel, reach with hands and arms
  • Climb or balance, stoop, kneel, crouch, or crawl
  • Talk or hear; vision; interact with co‑workers, residents or vendors
  • Occasional weekend, evening or night work if needed to ensure shift coverage
  • On‑Call on an as needed basis
  • Possible exposure to communicable diseases and infections, latex, blood‑borne pathogens, various drugs, chemical infections, biological hazards
  • Requires travel occasionally
  • Ability to lift up to 50 pounds
Management and Decision Making

Makes standard and routine decisions based on detailed guidelines with little independent judgment; problems are generally solved with clear, detailed guidelines or reported to a supervisor.

Knowledge and Skills

Basic level of skill, follows written or verbal instructions; basic technical skills for repetitive and well‑defined duties. Basic knowledge of the organization and knowledge of PC's and word processing software in Microsoft Windows environment. Effective oral and written communication skills.
