Vista Prairie Communities in Willmar, Minnesota is seeking a Memory Care Manager RN who will develop and coordinate care programs for residents with memory loss. You will work in a compassionate environment, ensuring the health and safety of residents through assessments and service plans. This full-time position offers a Monday to Friday schedule from 8am to 5pm, with on-call responsibilities every four weeks.

As a Memory Care Manager RN with Vista Prairie Communities, you will live the values of Caring, Respect, Collaboration, Innovation and Stewardship. The Memory Care Manager‑RN is responsible for developing, coordinating, and implementing programs and services for residents with memory loss. In addition, the position will also ensure the health and safety of all residents through assessment and reassessment, development and implementation of appropriate service plans and plans of care, monitoring resident services, and supervision. The main responsibility is to provide a therapeutic environment that maintains each resident’s highest level of physical, social and physiological well‑being. Positon is MOnday through Friday, 8am - 5pm. On call every 4 weeks - rotating with other Nursing staff. On call responses primarily can be handled by phone with Direct Care staff.
