Job summary

Peregrine Senior Living is seeking a compassionate and organized LPN (MC) Assistant Case Manager to join our memory care team in Albany, NY. This part-time role provides administrative, documentation, coordination, and Licensed Practical Nurse support in collaboration with the Memory Care Director and Case Manager.

Qualifications

• Active LPN license in good standing.
• Experience in senior living/memory care preferred.
• Strong organizational and administrative skills.
• Ability to maintain confidentiality.
• Effective communication with residents and families.

Responsibilities

• Provide administrative and case management support to the Memory Care department.
• Assist with assigned case management tasks and follow-up activities for residents.
• Maintain accurate, timely, and organized resident documentation and records.
• Assist with gathering and organizing information for resident assessments and care planning.
• Help coordinate communication and follow-up with residents, families, and care team as directed.
• Support the Case Manager and Memory Care Director in monitoring resident needs and ensuring appropriate follow-up.
• Maintain documentation and records in accordance with company policies and applicable regulatory requirements.
• Collaborate with team members to promote resident well-being, dignity, rights, and quality of life.
