Job summary

Chandler Hall Health Services in Newtown, PA is seeking a Memory Care LPN to join our geriatric care team. The role involves clinical oversight, medication management, resident assessments, and coordinating care with physicians, families, and staff.

This full-time position requires rotating weekends and day/night shifts. You will supervise care staff, perform medication reconciliation, support safety, and respond to emergencies within your scope of practice.

Qualifications

• Active LPN license in good standing.
• Experience in memory care or geriatric nursing preferred.
• Ability to supervise care staff and coordinate care.
• Shift flexibility with rotating weekends and day/night shifts.
• Strong patient safety and infection control knowledge.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans.
• Perform medication reconciliation, medication reviews, and documentation.
• Promote a safe environment and identify risks related to falls, elopement, infection control, and resident behaviors.
• Respond appropriately to emergencies and initiate interventions within the scope of practice.
• Communicate effectively with residents, families, physicians, pharmacies, home health agencies, and other health professionals.
• Assist in training and monitoring of medication administration.
• Supervise and support care staff/med-techs in accordance with laws and policies.
• Maintain ongoing compliance with regulations.

Job Description:

• Rotating weekends, day/night shift
• The LPN provides clinical oversight, medication management, resident assessments, care coordination, staff supervision, and ongoing monitoring of residents.
• The LPN works collaboratively with caregivers, medication technicians, physicians, families, and other healthcare professionals to promote resident safety, dignity, independence, and quality of life.
• In the essential duties and responsibilities – provide direct nursing care to residents in accordance with applicable laws, regulations, facility policies and individualized care plans
• Perform medication reconciliation, medication reviews, controlled substance counts, and medication documentation as required.
• Promote a safe environment and identify potential risks related to falls, elopement, medication access, infection control and resident behaviors
• Respond appropriately to emergencies and initiate appropriate interventions within the scope of practice
• Communicate effectively with residents, families physicians, pharmacies, home health agencies, hospice providers and other members of the healthcare team
• Assist in training and monitoring of medication administration
• Perform resident assessments
• Supervise and support care staff/ med-techs in accordance with applicable laws, regulations, & facility policies
• Maintain ongoing compliance with chapter code 2600 regulations
