Grace Pointe Senior Care community offers a 26 bed, secured memory care unit and is now looking for a Memory Care Program Coordinator. The primary purpose of this position is to coordinate all functions of the Memory Care Program and is accountable for the physical, personal and emotional care of each resident.
The schedule for this position would consist of 8 hour shifts,10am-6:30pm, Thursday - Monday. The wage range for this position is $24 - $35 per hour and is dependent on experience and qualification.
Ensure coordination of nursing services with other resident services to ensure the continuity of the residents' total regimen of care.
Participate in facility surveys (inspections) made by authorized government agencies and plans of correction, as necessary.
Implement, supervise and participate in a daily, or more often if needed, review of the PointClickCare Dashboard(s) to ensure that clinical and interdisciplinary services are carried out in accordance with the Mission, plan of care, the standard of care and regulations.
Supervise the day-to day activities of the Resident Care Assistants and QMAPs on the Memory Care Unit.
Communicate effectively with medical staff, nursing personnel, and other department supervisors.
Have the ability to make independent decisions when circumstances warrant such action.
Must have the ability to plan, organize, develop, implement, and interpret the program’s goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Must demonstrate patience, tact, a cheerful disposition and enthusiasm.
Coordinate admissions to include the assessment process and determination of the appropriate level of care. Ensure physician admission orders are received. Create Service Plans for residents and revise per facility policy and regulations.
Assess assisted living residents per regulation
Meet with Resident/Family/Responsible Party to discuss Service Plan and level of care. Revise Service Plans after incidents / occurrences.
Ensure all Resident charts are audited regularly for accuracy.
Meet daily with Assisted Living staff, altering times to meet with all shifts, and utilize the shift communications in PCC to be aware of resident changes in condition.
Provides education/training for the Memory Care Program staff, including new employee orientation. Supervises all Memory Care staff.
Participate in facility committees as directed by the Director of Nursing, i.e., Safety Committee, QA Committee, etc.
Have an awareness of the safety and sanitation requirements to provide an environment that is optimum for employees to work in and residents to live in.
Coordinate healthcare services (home health, hospice, etc.) maintaining consistent contact with physicians.
Coordinate discharges for residents whose needs can no longer be met in Memory Care/Assisted Living.
Demonstrate flexibility in adjusting to change, resident needs and customer expectations; meet company standards relating to deadline and attendance.
Maintain an excellent working relationship with other departments.
Maintain a liaison with the residents, their families, support departments, etc., to adequately plan for the residents' needs.
May be needed to assist in the evacuation of residents during emergency situations.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment, throughout the facility.
