Job summary

Sycamore Heights Health and Rehabilitation in Louisville, KY is seeking a compassionate Memory Care CNA to join our Memory Care Unit. You will provide patient, person-centered care to residents with Alzheimer's and memory-related conditions, promoting dignity, independence, and comfort in a structured, safe environment.

You will perform ADLs, assist with mobility, monitor health, document care, and communicate with nurses and families.

Qualifications

• Current CNA license in the state.
• At least one year of LPN experience in a long-term care setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Provide hands-on care to residents, including assistance with ADLs such as bathing, dressing, grooming, and toileting.
• Support residents with memory-related conditions using patience, redirection, and reassurance techniques.
• Assist with mobility, transfers, and ambulation to ensure resident safety.
• Monitor and report changes in condition, behavior, or mood to the nurse.
• Obtain and record vital signs as directed.
• Assist with meal service and feeding, encouraging independence.
• Maintain a safe, calm, and structured environment for residents.
• Provide emotional support and companionship, building meaningful relationships.
• Adhere to infection control guidelines and confidentiality.
