Job summary

Emory Healthcare is seeking an experienced Advanced Practice Provider (APP) to join the Integrated Memory Care team in Georgia. The APP will conduct primary care visits, cognitive assessments, care planning visits, annual wellness visits, and more, with prior dementia care experience preferred.

• Completion of an accredited Nurse Practitioner or Physician Assistant program.
• Licensed as an advanced practice nurse in Georgia and certified as a Nurse Practitioner by ANCC or equivalent, or Certified as a Physician Assistant by NCCPA.
• American Heart Association BLS and Healthcare Provider certifications required; ACLS may be required in some areas.

Responsibilities

• Assists in the evaluation and treatment of patients.
• Documents the delivery of care in the individual's medical records.
• Conducts patient interviews and documents patient histories, psychosocial histories, health status and medical problems or potential problems.
• Under the direction of a physician, performs physical examinations and specialized diagnostic and/or therapeutic procedures including cast application and removal, wound suturing and dressing changes.
• Assists physician in diagnosing medical problems, developing care plans, and prescribing treatment.

Integrated Memory Care is a nationally-recognized primary care clinic for people living with dementia. The IMC is seeking a nurse practitioner who can work effectively in a high-functioning, interdisciplinary team.

The APP will conduct new and established primary care visits, cognitive assessment and care planning visits, annual wellness visits, and more. Prior experience working with older adults or people living with dementia is preferred.

Minimum Qualifications

• Completion of an accredited Nurse Practitioner program or Physician Assistant program.
• Currently licensed as an advanced practice nurse in the state of Georgia by the Georgia Board of Nursing and certified as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or equivalent body, or Certified as a Physician's Assistant by the National Commission on Certification of Physician Assistants (NCCPA).
• Licensed as appropriate in the state of Georgia.
• American Heart Association BLS and Healthcare Provider certifications required. ACLS may be required in some areas.
