Job summary

Highmark Inc. is seeking a Health Outreach liaison to serve as a bridge between members, the Health Plan, and regulators. You will perform telephonic support to assess member needs, schedule appointments, and educate members about their rights and responsibilities.

You will also assist with navigating grievances and appeals and advocate for member interests. The role includes proactive outreach, enrollment into disease management programs, and coordination with providers and community agencies.

Qualifications

• 1 year in Community Outreach or Customer Service.
• Familiarity with healthcare and/or insurance.
• Experience with Medicaid and/or Medicare population.
• Strong communication and active listening.

Responsibilities

• Communicate effectively with staff, providers and members to accomplish business and member needs.
• Initiate outreach by telephone for preventive health initiatives.
• Educate members on preventive health services and rights.
• Recruit and enroll members into Disease Management Programs as appropriate.
• Assess opportunities for Care Management intervention and make referrals.
• Coordinate access to preventive health services with members, providers and community agencies.
