Job summary

Wayspring is hiring a Member Engagement Specialist, FES in Chicago, IL. The role focuses on initial engagement and enrollment of eligible health plan members within treatment facilities or community settings, plus ongoing support through the program.

Travel to the greater Chicago area is required, with mileage reimbursement provided. The ideal candidate has 3+ years in outreach or social work, strong communication skills, and experience with CRM/EHR systems.

• Incorporate recovery support themes to enhance member experience.
• Review member profiles for SDoH and clinical updates.

Responsibilities of the Member Engagement Specialist, FES

• Facilitate s in person enrollments at Wayspring partnered facilities, and where appropriate, conduct outreach to providers to secure member referrals
• Engages members and provide s them with an educational overview of Wayspring's service offerings, with the goal of program enrollment
• Secure s verbal and written consent, and obtain appropriate paperwork for services including consent to clinic services and release of information forms from members to participate in Wayspring's services
• Collaborate s with facility discharge planning teams to ensure members leave facilities with a safe and stable plan
• Secure s members first appointment with clinical services at Wayspring
• Provide s follow-up to members for 30-90 days post discharge at partnered facility, driven by the individual member's needs.
• Initiate s creative strategies to facilitate member contact, including in-person opportunities
• Provide s ongoing engagement to members both telephonically and in-person
• Incorporate s recovery support themes into engagement to improve member experience
• Regularly review s member profile for SDoH and clinical updates
• Maintain s performance accountability around member enrollment, clinic conversion, and achieve performance targets
• Adhere s to Wayspring information security and privacy requirements

Requirements

• Minimum of three (3) years of experience in an outreach, social work, or street outreach role
• Highly organized, detail-oriented, and self-driven with the ability to work independently and as part of a team
• Demonstrated ability working in a matrixed environment
• The ability to communicate effectively and persuasively - including de-escalation skills and crisis management - is required
• Exceptional verbal and written communication skills, and effective presentation skills
• Experience with CRM platforms or Electronic Health Record systems (EHR/EMR) is highly preferred
