Qualifications

• Bachelor's Degree of Pharmacy from an accredited school of pharmacy.
• State licensure as a Pharmacist.
• 2 years post graduate experience.

Responsabilités

• Review medication safety events and assign follow-up to designated pharmacy stakeholders.
• Identify trends in medication safety events and propose process improvements for the region.
• Collaborate with stakeholders to develop action plans addressing safety concerns.
• Track follow-up and loop closure on significant events with the Manager of Quality and Medication Use Systems.
• Serve as a consultant for medication safety to embed safeguards during workflow changes and policy updates.
• Collaborate with the medication safety team on system projects and initiatives.
• Operationalize the system medication safety strategy locally with the Manager of Quality and Medication Use Systems.
• Participate in CCRs, RCAs, and FMEAs focused on medication management opportunities.

Formation

Bachelor's Degree of Pharmacy from an accredited school of pharmacy.

State licensure as a Pharmacist.

2 years post graduate experience.
