Job summary

Responsibilities

  • Conducts timely, professional, and structured medication history interviews with patients, caregivers, family members, or other appropriate sources during the admission process.
  • Obtains and documents a complete list of medications the patient was taking prior to admission.
  • Verifies key medication details, including medication name, strength, dose, route, frequency, last dose taken, indication when appropriate, adherence concerns, and source of information.
  • Identifies and documents medication allergies, intolerances, reaction details, and patient reported adverse drug reactions.
  • Compares patient reported information against available medication records and identifies discrepancies requiring pharmacist or provider review.
  • Identifies potential medication safety concerns, including duplicate therapy, unclear dosing, nonadherence, expired medications, and therapeutic omissions.

JOB SUMMARY

The Medication Reconciliation Technician is responsible for obtaining, verifying, and documenting the most accurate possible medication history for patients upon admission. This role within the pharmacy department supports patient safety, continuity of care, and clinical decision making by ensuring that the patient's home medication list is accurate before inpatient medication orders are finalized or reconciled by the licensed provider and pharmacist.


Working under the supervision of a licensed pharmacist, the medication reconciliation pharmacy technician conducts structured patient and caregiver interviews, reviews available medication records, contacts outpatient pharmacies, prescribers, or patient caregivers when needed. And documents medication information in the electronic health record. This position will play a critical role in reducing medication discrepancies, omissions, duplications, dosing errors, and avoidable medication-related harm during transitions of care.


This position exists to strengthen medication safety at the point of admission. This position will report to the pharmacy technician supervisor.



ESSENTIAL DUTIES AND RESPONSIBILITIES


  • Other duties may be assigned.

  • Conducts timely, professional, and structured medication history interviews with patients, caregivers, family members, or other appropriate sources during the admission process.

  • Obtains and documents a complete list of medications the patient was taking prior to admission.

  • Verifies key medication details, including medication name, strength, dose, route, frequency, last dose taken, indication when appropriate, adherence concerns, and source of information. The technician must clarify unclear, incomplete, or conflicting medication information.

  • Identifies and documents medication allergies, intolerances, reaction details, and patient reported adverse drug reactions.

  • Compares patient reported information against available medication records and identifies discrepancies requiring pharmacist or provider review.

  • Identifies potential medication safety concerns, including duplicate therapy, unclear dosing, nonadherence, expired medications, and therapeutic omissions.



EDUCATION, EXPERIENCE AND REQUIRED QUALIFICATIONS


  • Must be a Registered Pharmacy Technician in the State of Florida.

  • Minimum 1 year of hospital pharmacy experience.

  • Strong verbal communication skills. This position will encounter situations where they must communicate with patients who are ill, confused, or possibly hearing impaired.

  • Ability to work independently within defined scope while appropriately escalating clinical or safety concerns to a pharmacist.

  • Ability to stand, walk, and use a computer or workstation on wheels for an extended period of time

  • Ability to work rotating shifts.



DESIRED QUALIFICATIONS


  • Prior experience as a medication reconciliation technician.

  • High working knowledge of medications names (brand/generic), common dosing schedules, and pharmacy terminology.

  • Prior experience with Epic electronic health record

  • Ability to communicate in Spanish
