Job summary

Blessing Health is seeking a Data Clerk to collect and document patients' home medication lists as they are admitted to Blessing Hospital. You will interview patients or caregivers and gather medications from multiple sources, then work with the pharmacist to complete an initial medication review.

Required are a high school diploma and pharmacy technician training; Illinois license or certification is preferred.

Qualifications

  • High School Diploma or equivalent is required.
  • Completion of Pharmacy Technician Training Course is required.
  • Computer skills are required.
  • Excellent written and verbal communication skills.

Responsibilities

  • Collect the current home medication list for patients admitted to Blessing Hospital.
  • Gather information from interviews, home medication lists, and external records.
  • Document the medication list and collaborate with the pharmacist for initial review.
  • Assist with accuracy of home medication reconciliation in the non-hospital setting.

Education

High School Diploma or equivalent
Completion of Pharmacy Technician Training Course
Illinois Pharmacy Technician license
NPTCB Certification or equivalent
Excellent written and verbal communication
Excellent typing skills
Computer skills

Job Summary

This position is responsible for collection of the current home medication list of patients admitted to Blessing Hospital. This list of medications would include prescription medications and nonprescription (over the counter) medications currently administered to the patient in the non-hospital setting.

  • The Data Clerk will gather this medication information from:
    • Interview of the patient and/or significant other
    • The patient’s home medication list
    • Home medications in the possession of the patient
    • Long term care medication administration record
    • Physician clinic list
    • Retail pharmacies, as available.
  • The Data Clerk will document the list of current medications and work with the pharmacist to complete an initial review.
Education/Training/Experience

JOB QUALIFICATIONS

Required

  • High School Diploma or equivalent.
  • Completion of Pharmacy Technician Training Course.
  • Computer skills are required.
  • Excellent written and verbal communication skills.

Preferred

  • Pharmacology education or pharmacy experience preferred.
  • Excellent typing skills preferred.

License/Certifications/Special Requirements

  • Illinois registration as a Pharmacy Technician
  • Certification by the National Pharmacy Technician Certification Board or equivalent unless exempted by the state of Illinois due to Grandfathering

PAY STATUS

  • Non-exempt hourly
