The pharmacy medication reconciliation technician is a certified pharmacy technician who supports pharmacy services by obtaining medication and allergy histories for patients admitted to the hospital. Information is gathered by interviewing the patient, family members, calling pharmacies, long-term care facilities, and other providers. This information is communicated to pharmacists and healthcare providers to determine treatment plans and medication orders during hospitalization and after discharge.

Requirements:

• If previously registered, registration must be active upon hire.
• National certification as a pharmacy technician.
