Job summary

Naples Comprehensive Health - NCH is seeking a Medication Reconciliation Technician to obtain and verify accurate medication histories for patients at admission. You will support patient safety by reconciling home medications prior to inpatient orders under a pharmacist's supervision.

In this role, you will interview patients and caregivers, review records, contact outpatient pharmacies when needed, and document in the EHR. The position strengthens medication safety during transitions of care.

Qualifications

• Must be a Florida-registered Pharmacy Technician.
• Minimum 1 year hospital pharmacy experience.
• Strong verbal communication skills; ability to interact with ill or confused patients.
• Ability to work independently within defined scope and escalate safety concerns to a pharmacist.
• Ability to stand, walk, and use a computer workstation for extended periods.
• Ability to work rotating shifts.
• Desirable: experience as a medication reconciliation technician.
• Experience with Epic EHR is a plus.
• Spanish language ability is a plus.

Responsibilities

• Conducts structured medication history interviews with patients, caregivers, or family during admission.
• Obtains and documents a complete medication list prior to admission.
• Verifies medication details (name, strength, dose, route, frequency, last dose, indication, adherence, source).
• Identifies and documents allergies and adverse reactions.
• Compares patient-reported information with records and flags discrepancies for pharmacist/provider review.
• Identifies potential medication safety concerns (duplicate therapy, dosing issues, nonadherence, expired meds).
