Job summary

Adventist Health Lodi Memorial in Lodi, CA seeks a Pharmacy Technician to support the inpatient pharmacy. You will assist with reconciliation of home medications with those prescribed during admission and help ensure accuracy at discharge.

Under supervision, you will perform essential tasks, generate data for outcome analysis, and support senior staff in daily operations. CA license required; high school diploma or equivalent; associate degree preferred; board certification preferred.

Qualifications

• High School Education/GED or equivalent: Preferred
• Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
• State Pharmacy Technician license: Required in CA and OR
• Pharmaceutical Technician Board certification: Preferred

Responsibilities

• Assists the patient’s physicians to reconcile discrepancies between home medications and medications prescribed during the inpatient stay.
• Provides assistance with medication reconciliation at discharge for high risk patients by identifying and communicating medication discrepancies to prescribers for resolution.
• Generates and analyzes quantitative data on the effectiveness of medication reconciliation for analysis of regulatory compliance, clinical and economic outcomes.
• Assists pharmacy staff as requested under the direction of the senior technician, lead technician or director.
• Performs other job-related duties as assigned.
