Job summary

Emory Healthcare is seeking a Medication Reconciliation Assistant to work under supervision of a pharmacist, collecting and documenting the best possible medication list for patients across the hospital. You will interview patients, families, and external providers to ensure accuracy and completeness.

Qualifications

• Two years of college.
• Successful completion of an ASHP-accredited technician training program and current CPhT certification or enrollment as a Georgia Pharmacy Intern.
• Two years of experience as a Pharmacy Technician.
• Current registration with the Georgia Board of Pharmacy.
• Must maintain certification and registration through the length of employment.

Responsibilities

• Interview patients, families, and external providers to collect and verify the medication list.
• Document the best possible medication history in the electronic medical record.
• Coordinate corrections and communicate updates to physicians and pharmacists.
• Assist in training new personnel and participate in quality improvement initiatives.
• Support medication safety and patient care by timely data collection and reporting.
