Job summary

Beacon Health System in Kalamazoo, MI seeks a Medication Reconciliation Technician who interviews patients to obtain and document comprehensive medication histories, including allergies, under supervision of a Pharmacist. You will compile and reconcile home medication lists across multiple sources and document findings in the hospital system.

The role includes mentoring new technicians, collecting data for quality improvement, and maintaining accurate logs and records to ensure safe patient

Qualifications

• The knowledge, skills, and abilities include a high school diploma/equivalent and a MI-approved pharmacy tech training program.
• Current Michigan State Board license as Pharmacy Technician and CPhT certification are required.
• Minimum of two years of pharmacy technician experience; outpatient/retail experience preferred.
• Must be at least 18 years old.

Responsibilities

• Interview patients/families to obtain medication histories and allergies.
• Document medications, dosages, routes, and last doses in the patient care system.
• Identify discrepancies and escalate to a pharmacist for clarification.
• Reconcile home medication lists with other sources (physicians, pharmacies, facilities).
• Mentor/train new Pharmacy Technician-Medication Reconciliation Technicians.
• Collect data, perform quality monitors, and maintain records.
