Job Details

The Medication History Pharmacy Tech reports to the Assistant Director of Pharmacy and is responsible for accurately collecting and entering current patient medications into the hospital's information system for use by physicians and providers involved in patient admission and discharge. The primary duties include conducting patient interviews and reviewing internal and external systems for relevant information. The Medication History Pharmacy Tech must relay any unresolved transition of care issues to the lead pharmacist and/or physician. Additionally, the role involves performing limited medication management and distribution activities under pharmacist supervision, adhering to department policies, procedures, standards, and regulations within specified time frames.

Job Description

Experience:

  • At least 1 year of Pharmacy Technician experience.
  • Preferred experience in medication history/reconciliation.
  • Retail pharmacy experience is a plus.

Education & Training:

  • High School Diploma or equivalent required; Licensed Practical Nurse or Bachelor's degree preferred.

License, Certification & Registration:

  • Certified Pharmacy Technician (CPhT); if LPN, CPhT required within 1 year of hire.
