Job Details

The Medication History Pharmacy Tech reports to the Assistant Director of Pharmacy and is responsible for accurately collecting and entering current patient medications into the hospital’s information system for utilization by physicians and providers responsible for admitting and discharging patients. The primary duties include patient interviews and reviewing internal and external systems for relevant information. The Tech is responsible for relaying any outstanding transition of care issues to the lead pharmacist and/or physician. The Tech may also perform limited medication management and distribution activities under supervision, following department policies, procedures, standards, and regulations within designated time frames.

Experience

• 1+ years of Pharmacy Technician experience.
• Prior experience with medication history/reconciliation preferred.
• Retail pharmacy experience preferred.

Education & Training

• HS Diploma or equivalent required; Licensed Practical Nurse or Bachelor’s degree preferred.

License, Certification & Registration

• Certified Pharmacy Technician (CPhT); if LPN, CPhT required within 1 year of hire.
