Job summary

St Joseph's Health and Trinity Health seeks a Part-time Medication History Technician to conduct patient interviews across hospital settings — pre-admission testing, ED, or procedural areas — gathering comprehensive medication histories for EMR entry.

Responsibilities

• Identify the patient to be seen per hospital policy based on alerts and consults.
• Verify and document patient allergies and reported reactions.
• Complete medication history using established questionnaire to obtain comprehensive list of medications (name, dose, route, freq, last dose).
• May involve interviewing patient, caregiver or family member.
• Document each medication in the electronic medical record.
• Document dates and times of last doses per protocol.
• Screen for vaccination history.
• Confirm medication history using secondary sources (e.g., EMR, MARs, pharmacy, physician offices).
• Knowledge of hospital polices, accountability and compliance.
• Train and precept other technicians, pharmacists and students.

Summary :

A medication historian technician conducts patient interviews within the hospital, in pre-admission testing, emergency department or in procedural areas to obtain medication lists. A standardized questionnaire and process to obtain an extensive medication history is used for each patient. This history is a comprehensive list of prescription medications, infusions, over the counter products, immunizations, dietary and herbal supplements which is entered into the electronic medical record. The medication historian technician will use a multitude of resources to gather patient medication information including calling physician offices, dialysis centers, pharmacies, hospitals and nursing homes and obtaining medication administration records to confirm outpatient regimens.

Job Qualifications

Minimum Education: high school diploma or equivalent. Minimum of 1 year pharmacy experience. Must become nationally certified within 1 year of employment. Strong communication and leadership skills. Accurate and detail oriented with exemplary time management skills. Knowledge of brand and generic medication names and dosing for the top 100? 200? commonly prescribed medications within 90 days of hire. Must be able to write, speak and read English proficiently. Ability to interact with patients and their caregivers as well as various members of the healthcare team (i.e., nurses, pharmacists, physicians, outpatient providers, nursing home staff). Position requires long periods of standing and significant walking.

• Complete medication history for each medication identified using an established questionnaire as a guide to obtain a comprehensive list of patient’s entire medication regimen (i.e., medication name, dose, route, frequency, time/date last dose taken).
• This may involve interviewing the patient, caregiver or family member.
• Document each medication, including prescription, outpatient infusions, over-the-counter medication, dietary and herbal supplements into the electronic medical record.
• Documents dates and times of last doses per protocol.
• Screen for vaccination history.
• Confirm the medication history using a secondary source (i.e., electronic medication history resource, outpatient pharmacy, MARs, physician office, medication bottles...etc).
• Knowledge of hospital-wide polices, accountability and personal compliance.
• Training and precepting other pharmacy technicians, pharmacists and students.

Pay Range: $19.20 - $27.85

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
