Job summary

Responsibilities

• Document medications in the electronic medical record, including prescriptions and infusions.
• Screen for vaccination history.
• Confirm the medication history using secondary sources (electronic history resources, MARs, pharmacies, offices).
• Train and precept other pharmacy technicians, pharmacists and students.

  Summary

  A medication historian technician conducts patient interviews within the hospital, in pre-admission testing, emergency department or in procedural areas to obtain medication lists. A standardized questionnaire and process to obtain an extensive medication history is used for each patient. This history is a comprehensive list of prescription medications, infusions, over the counter products, immunizations, dietary and herbal supplements which is entered into the electronic medical record. The medication historian technician will use a multitude of resources to gather patient medication information including calling physician offices, dialysis centers, pharmacies, hospitals and nursing homes and obtaining medication administration records to confirm outpatient regimens.

  Job Qualifications

  Minimum Education: high school diploma or equivalent. Minimum of 1 year pharmacy experience. Must become nationally certified within 1 year of employment.   Responsibilities

  Pay Range: $19.20 - $27.85

  Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
