Qualifications

• Associate’s degree plus 2 years of qualifying experience or a Bachelor’s degree plus 1 year.
• Valid driver’s license, safe driving record, valid auto insurance and access to a vehicle required.
• Qualifying experience includes verifiable full- or part-time case management for individuals with mental illness, homelessness, or chemical dependence.
• An alternative combination of education, credentials, and experience may be substituted for a degree.

Responsibilities

• Provide oversight and monitoring of client medications at homes of identified clients.
• Link residents with appropriate resources.
• Assess client needs to determine frequency of medication monitoring.
• Provide education on medication and health disorders.
• Prepare clients to monitor and take medications independently.
• Travel to apartment sites within the Mental Health Licensed Program and coordinate with prescribers.

Salary Range: $41,860 ($23.00/hr.) - $54,600 ($30.00/hr.)

The above salary range represents Options for Community Living’s good faith and reasonable estimate of potential compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future. When determining a salary offer, several factors may be considered as applicable (e.g., years of relevant experience, education level, language skillset, credentials, professional licensure, budget, and internal equity).

Schedule: Monday, Wednesday, Friday: 9:00 AM - 5:00 PM and Tuesday, Thursday: 12:00 PM - 8:00 PM (35 hours/week)

Location: In person; Ronkonkoma site with field visits required.

Responsibilities:

The Medication Case Manager provides various support services to residents with psychiatric disabilities in the Apartment Treatment Program. Reports to the Program Supervisor.

• Provide oversight and monitoring of client medications at the homes of identified clients who are in need of this service.
• Link residents with appropriate resources.
• Engage in ongoing assessment of client needs to determine frequency of medication monitoring.
• Provide education and resource information regarding medication and medical and mental health disorders.
• Prepare clients to monitor and take their medications independently.
• Must adhere to Options’ medication policies and procedures.
• Complete timely submission of medication management progress notes, assessments and other relevant documentation in compliance with department and agency standards.
• Conduct overall assessment of client welfare and appropriate intervention and follow-up as needed.
• Participate in on-going communication with medication prescribers, service providers, Case managers, and any other collateral contacts as needed.
• Provide training on medication management and supervision for Licensed Program staff.
• Must travel to apartment sites within the Mental Health Licensed Program. Must use personal vehicle or agency vehicle when available to travel to sites.
• Assist with medication count supervision across apartment treatment program sites when needed.
• Meet with the Program Supervisor at least once per week to discuss client progress.
• Maintain a case load and perform the duties of a full-time residential counselor as needed.
• Any other relevant tasks as assigned by management.

Salary Range

$41,860—$54,600 USD
