Qualifications

  • Bachelor’s degree in applicable field or equivalent combination of education and experience.
  • Two years of related experience in public health, health outreach, or health education.
  • Experience working with healthcare organizations, providers, patients, or community-based organizations.
  • Community Health Worker Certification or equivalent.

Responsibilities

  • Coordinate program activities and travels to provide on-site support at multiple locations.
  • Serve as first point of contact for program information and respond to requests.
  • Conduct resident interviews and assist with pharmaceutical applications.
  • Build positive relationships with patients, providers, and community organizations.
  • Confer with Healthy South Texas staff regarding patient care plans.
  • Maintain databases and program materials, including records and correspondence.
  • Edit and prepare reports, publications, and presentations.
  • Keep knowledge of community resources up-to-date and review program policies.
  • Perform other duties as assigned.
  • Assist patients in reviewing care plans and encourage engagement.
  • Help patients overcome barriers to care and use community resources.
  • Provide culturally appropriate support in visits and follow-ups, including home visits.

What we want The Program Assistant, under general direction, provides standard program support and coordination of activities for the Medication Assistance Program. What you need to know Salary: $18.20 hourly Travel: This position may require up to 20% or more of travel. Schedule: This position may require work beyond normal office hours and/or on weekends.

Medication Assistance Program
  • Assists in coordinating program activities and travels to various program locations to provide on-site support.
  • Serves as first point of contact for program information and coordinates responses to request for program information as needed.
  • Conducts personal interviews with residents and aids in completing pharmaceutical applications for assistance programs and follows up on pending applications.
  • Builds and maintains positive working relationships with patients, healthcare provides, and community organizations.
  • Confers as needed on a timely basis with Healthy South Texas staff regarding patient care plans.
Administrative
  • Maintains pharmaceutical company and client databases and other program materials, including administrative, fiscal, and client records.
  • Composes correspondence, reports, publications, presentations, including editing/proofreading program materials as needed.
  • Continuously expands knowledge and understanding of community resources, services, and available programs.
  • Assists in reviewing and updating program policies and procedures and makes recommendations for program improvement.
  • Performs other duties as assigned.
Patient Support
  • Assists patients in reviewing care plans and results in an effective manner.
  • Motivates patients to be active and engaged participants in their health.
  • Assists patients in overcoming barriers to obtaining needed medical care and social services and utilizing other community resources.
  • Provides culturally appropriate support and information for the patient and their families in a series of structured visits and follow-up phone calls in a variety of settings, including the patients’ homes.
Qualifications
Required Education And Experience
  • Bachelor’s degree in applicable field or equivalent combination of education and experience
Preferred Qualifications
  • Ability to speak English and Spanish
  • Two years of related experience in public health, health outreach, or health education.
  • Experience working with healthcare organizations, providers, patients, or community-based organizations.
  • Community Health Worker Certification or equivalent.
