This job assists providers with the refill process by ensuring patient compliance with medication, lab monitoring, adherence to therapy by providing appropriate refill amounts on medication refill requests, maintaining accurate pharmacy preferences, and identifying discrepancies or problems with medication history. It involves communicating with patients, families, caregivers, healthcare providers/practitioners, and pharmacists to obtain an accurate and complete medication history for patients while documenting encounters within the electronic health record (EHR). Any issues are forwarded to a pharmacist for resolution.

Education

Required – High School Diploma or equivalent.

Work Experience

Required – 2 years of experience as a pharmacy technician.

Preferred – Electronic prior authorization experience, experience taking patient medication histories.

Certifications

Required - Medication Therapy Management Certification obtained through the PTCB or NHIA Reimbursement training, or the Medication History Certification from the American Society of Health-System Pharmacists (ASHP) within 6 months.

Preferred - Certified Pharmacy Technician license in the state of practice.

Knowledge, Skills, and Abilities (KSAs)
  • Proficiency in using computers, software, and web-based applications.
  • Knowledge of pharmaceutical mathematics.
  • Ability to operate standard pharmacy and office equipment.
  • Effective verbal and written communication skills, with the ability to present information clearly and professionally.
  • Strong interpersonal skills.
Job Duties
  • Obtain and clarify patients' home/chronic medication lists.
  • Interview patients and/or family members to ensure proper identification of doses and drugs.
  • Use the EHR to input patients’ medication lists.
  • Communicate discrepancies identified during medication history with the pharmacist and clarify with the patient and prescribers as needed.
  • Collect patients’ prescription benefit information.
  • Direct all questions or inquiries outside the scope of medication history to appropriate caregivers or staff.
  • Provide high-quality customer service to patients, families, and care team members.
  • Document findings into the EHR for collaboration amongst providers and staff.
  • Adapt behavior to the specific patient population, respecting privacy and communication preferences.
  • Perform other related duties as assigned.

Medium Work - Exerting 20 to 50 pounds of force occasionally, 10 to 25 pounds frequently, or greater than negligible up to 10 pounds constantly to move objects. Physical demands are in excess of sedentary work. Routine involves no exposure to blood, body fluids, or tissue, and no emergency care or first aid is required. There may be occupational risk for exposure to hazardous medications or waste, depending on duties.
