The Medicare Wellness Coordinator collaborates with primary care providers and the healthcare team to enhance patient care for Medicare recipients. This role involves conducting wellness assessments, coordinating Annual Wellness Visits (AWVs), supporting chronic care management, and ensuring compliance with Medicare guidelines. The coordinator engages with patients, providers, and administrative staff to promote preventive care, improve health outcomes, and enhance the overall patient experience. This position requires a strong understanding of the nursing process, critical thinking skills, and the ability to plan and coordinate care effectively. The role involves handling confidential patient information and requires maintaining professionalism when interacting with patients, families, visitors, and clinic staff.

Qualifications
Current Licensure of a Registered Nurse in Arkansas or a state that is included in compact licensure agreement.
Strong organizational and time management skills, with the ability to prioritize multiple tasks and role components.
Able to work in a demanding setting.
Strong computer skills
This is a Safety Sensitive position(the use of a Medical Marijuana card is prohibited according to Act 593).
