Job summary

Optum NV in Las Vegas seeks a Physician to join our Neighborhood Physician team. The role focuses on primary care within a high-acuity Medicare Advantage panel, combining clinic and home-based care to reduce hospitalizations and improve outcomes.

You will collaborate with APCs, case management, and social work to address medical and psychosocial needs while supporting quality initiatives and longitudinal care management.

Qualifications

• MD or DO with board certification or eligibility in Internal Medicine or Family Medicine.
• Active Nevada medical license and DEA (or ability to obtain).

Responsibilities

• Manage a panel of complex Medicare patients as PCP, overseeing all aspects of care.
• Provide care in clinic and home settings with APC support for field visits.
• Collaborate with interdisciplinary team to optimize care plans and transitions.
• Review and coordinate care for high-risk patients, including post-discharge follow-up and chronic disease management.
• Partner with clinical quality teams to meet HEDIS, RAF, and documentation standards.
• Maintain accurate, timely documentation in the EMR.
• Participate in team meetings and contribute to care model development and workflow optimization.
• Provide clinical guidance and collaboration with APCs.
