• High School diploma or GED required.

• CPR/AED certification required within 90 days of hire.
• Excellent interpersonal and communication skills.

Responsibilities

• Administer daily activities for medically integrated programs and track referrals.
• Collaborate with fitness staff to tailor exercise programs to assessments.
• Ensure compliance with physician approvals and clearance procedures.
• Perform outreach to healthcare providers to develop referral pathways.
• Serve as communications liaison among center and referral sources.
• Assist with membership transitions after program completion.
• Meet volume and financial budget expectations for programs.
• Provide participant reports to referring providers.
