Medical Technician Partner (MTP) - Paramedic

We provide hands-on social and medical care to our patients, both within care facilities and in their homes, focusing on community-building and tailored care experiences for seniors. What is this role about?

Our senior patients face social challenges called “Social Determinants of Health” (SDOH), such as lack of transportation. We have built a model that ensures consistent access to healthcare by providing clinical care both in clinics and in patients’ homes. The Medical Technician Partner (MTP) will deliver in-home care, support our patients, and serve as the eyes, ears, and hands of our medical teams, collaborating with primary care providers, Gather Guide Partners, and clinic administrators to deliver compassionate, equitable care.

Day-to-day duties include:

  • Providing in-home care and acting as a clinical liaison with primary care providers
  • Performing 6-8 home visits daily, addressing medical decompensation and care transitions
  • Evaluating CHF and COPD exacerbations through physical exams and vital signs assessment
  • Organizing inpatient or ED admission avoidance plans
  • Conducting social visits and occasional ED meetings
  • Calling patients to follow up on medications
  • Accessing medical control via the Clinical Medical Director when needed
  • Documenting patient notes in Athena Electronic Health Record

Requirements:

  • At least 1 year of experience as a Paramedic or in direct patient care
  • Active Paramedic certification in Massachusetts
  • Valid driver’s license with a safe driving record
  • CPR certification (AHA Healthcare Provider)
  • Strong English verbal, written, and reading skills
  • Availability from 8am to 5pm, Monday to Friday
  • Willingness and ability to drive to patients’ homes (company vehicle provided)
  • Eligible to work for any employer in the U.S.

Preferred qualifications:

  • Experience with seniors and social needs
  • Basic office tools proficiency (email, Outlook)
  • Phlebotomy Technician certification (a plus)
